What is a Qualifying Life Event (QLE)?

Important Information About Adding a Newborn to Coverage:
- DO NOT WAIT to receive the Social Security Number or state birth certificate to add your newborn.
- You must call the Benefit Center or login and add your newborn child within 31 days of the childs birth.
- A hospital record of the childs birth indicating the date of birth, childs name, and parents name can also be used as substantiation documentation.
- If you are adding a newborn child, you only need the childs name and date of birth to add the newborn to your coverage. The newborns Social Security Number can be provided at a later date. After the newborn has been added, you will receive a request to provide the required documentation (the newborns hospital documentation or legal birth certificate) to support the newly added dependents eligibility. You can either upload that directly to your Benefits System profile, or submit it to the Veolia Benefit Center.
What is the Dependent Verification Process?
- A dependent verification is required when enrolling your dependents onto your benefit plans.
- Timeframe – You have 31 days from the date of the life event to enroll and verify your dependents.
- Review – Veolia Acceptable Forms of Documentation For Dependent Verification/Substantiation
- We suggest that you have your documentation available electronically (for example, take a picture of your marriage certificate) so you can upload it as you enroll.
- Upload your dependent documentation onto the Benefit Enrollment site and you will receive notification once the dependent has been approved for verification.
- The dependents will be marked as pending until the verification process has been completed.
- You will receive a confirmation that your dependent has been verified
