Early Exit:

If you participated in the plan and have experienced an “Early Exit” event and wish to sell your shares, please complete the Early Exit Form and send the completed form along with the required supporting documentation per the instructions on the form.

 

ComputerShare:

The Sequoia third party administrator, Computershare, has completed the mailing of participant logon credentials to those who participated in the program. As a reminder, two letters were mailed to each participant: one contains your Logon ID and the other contains your password/pin.

In addition, Computershare has also mailed all participants a Form W-9 Request for Taxpayer Identification Number and Certification. Among other things, this certification verifies your taxpayer identification number, whether or not you pay US taxes, and whether or not you are subject to back-up withholding. The US requires tax certifications be completed by participants for every account held in the US (bank or brokerage), otherwise an automatic 28% back-up withholding will take place on sales proceeds and dividends.  For in-depth information regarding the tax consequences of participating in the plan, please read through this local supplement.
